COVID-19 Self-Reporting Survey for Jobsites

The safety of our employees and customers are very important.  As the COVID-19 virus continues to spread we are adopting recommendations from the Center for Disease Control to limit the risk of exposure to employees and other contractors.  In an effort to do so, we are asking each of you to complete this short questionnaire daily.

If you answered yes to question 1or 2 do not work at this job site today. Follow CDC guidelines, you can find these by following this link – (https://www.cdc.gov/coronavirus/2019-ncov/if-you-are-sick/steps-when-sick.html). You will need to seek medical attention if necessary. You may return to work by following the AMS Return to Work Policy.

If you answered yes to question 3 do not work at this site today. Follow CDC guidelines, you can find these by following this link – (https://www.cdc.gov/coronavirus/2019-ncov/if-you-are-sick/steps-when-sick.html). You will need to seek medical attention if you have symptoms. Call ahead before you go to a doctor’s office or emergency room. Tell them about your recent travel and your symptoms. Please notify Human Resources of your possible exposure immediately. You will be unable to return to AMS for 14 days from your last exposure. If you are diagnosed with COVID-19, you will need clearance from a medical professional before returning to work. If you are not diagnosed with COVID-19 and have quarantined for 14 days, you may return to work by following the AMS Return to Work Policy.
